This role enables associates to work virtually full-time, except for required in‑person training sessions, providing maximum flexibility and autonomy.
Work Shift: Monday – Friday, 8:00 am to 5:00 pm CST or EST, with rotating on‑call.
Responsibilities- Provide primary and urgent health care via telephone and tele‑video modalities to patients receiving home and community‑based services through state Medicaid programs and other partners.
- Develop and implement clinical plans of care for adult patients facing chronic and complex conditions, including co‑morbid medical and mental health diagnoses.
- Perform history, physical exam, and diagnostics as needed, and develop treatment plans aligned with patient goals and current conditions.
- Identify and close gaps in care.
- Meet the patient’s and family’s physical and psychosocial needs with support from the inter‑disciplinary team.
- Educate patients and families on medication usage, side effects, illness progression, diet, nutrition, adherence, and crisis prevention.
- Coordinate with other clinical team members, patients’ physicians, and providers to ensure timely, consistent, and compliant care.
- Maintain accurate patient medical records in accordance with state regulations and company policy.
- Participate in continuing education required by state and certifying bodies; prescribe medication as permitted by state authority.
- MS in Nursing.
- Active, national NP certification.
- Valid, current, active, and unrestricted Family or Adult NP license in New York.
- Valid, current, active RN Compact license.
- At least 2 years of experience managing complex care cases.
- Experience working with Electronic Medical Records (EMR).
- Bilingual in English and Mandarin.
- DEA registration or eligibility.
- Active Medicaid licensure in New York.
- Experience managing complex cases for developmental disabilities and chronically ill patients.
New York. Salary range: $115,184 to $198,144, based on location, experience, education, and skill level.
