Job Summary
The Community Health Center of Buffalo, a Federally Qualified Health Center (FQHC), is actively seeking an experienced Family Planning Nurse Practitioner to provide comprehensive outpatient care to patients at our modern facility. Job Summary
The Community Health Center of Buffalo, a Federally Qualified Health Center (FQHC), is actively seeking an experienced Family Planning Nurse Practitioner to provide comprehensive outpatient care to patients at our modern facility.

Minimum Qualifications
- Licensed NYS, board-certified, Registered Professional Nurse Practitioner.
- Experience as a Family Planning Provider, including Long-Acting Reversible Contraceptives.
- Computer literacy with proficiency in MS Word, Excel, and Outlook.
Experience with eClinical Works is preferred.
- Active Nurse Practitioner license
- Certification in Family/Adult Health Nurse Practitioner
- In-depth knowledge of family planning and contraceptive methods
- Excellent communication and counseling skills
- Strong commitment to providing compassionate and nonjudgmental care
- Conduct comprehensive health assessments for individuals seeking family planning services
- Develop personalized family planning strategies based on individual patient needs
- Educate patients on various contraceptive options, including benefits and potential side effects
- Prescribe medications and contraceptives as needed
- Collaborate with interdisciplinary healthcare team members to ensure holistic patient care
- Maintain accurate and up-to-date medical records
- Adhere to all relevant laws and regulations governing family planning services
