We are seeking a Registered Nurse to provide high‑quality primary care to high‑risk populations, including our Healthcare for the Homeless program. This Monday‑Friday role requires case/care management knowledge and compassion for diverse patients.
About the RoleThe Registered Nurse – Community Health will use critical thinking and kindness to evaluate patient cases, devise solutions, and coordinate care. Most interactions will be over the phone, but you will also meet patients in various settings. You will collaborate with a clinical team to design individualized, patient‑centered care.
Responsibilities- Initiate and manage care plans; coordinate care and address barriers to adherence.
- Build trusting relationships to facilitate access to health‑care services.
- Help patients and families navigate Neighborhood services and external community resources.
- Provide health education at community events as a Registered Nurse.
- Participate in evening street/shelter outreach, assess needs, and encourage patients to establish care.
- Degree from an accredited two‑year nursing school.
- Current NYS Professional Registered Nurse license.
- Minimum 2 years of clinical nursing experience.
- CPR certification.
- Knowledge of case/care management principles.
- Strong attention to detail, organizational, project, and time‑management skills.
- Demonstrated kindness, compassion, and respect for patients.
- Resiliency and a problem‑solving mindset.
- Teamwork orientation and openness to diverse perspectives.
- Proficiency with Microsoft Office, Adobe, and database management.
Compensation starts at $36.50 per hour.

Registered Nurse Tuition Reimbursement Potential:
