Kaleida Health is seeking an Onsite Home Care Nurse to coordinate the total Plan of Care for patients, assess needs, plan interventions, implement care, and evaluate outcomes. You will lead the health care team to maintain high standards of nursing practice in the home care setting.
Applicants should have an Associate's Degree, current RN licensure, and 1–5 years of relevant home care experience. Work hours are 8:00 AM to 4:30 PM, bi-weekly pay, and on-site scheduling with no weekend requirement.
