RN Field Clinical Care Manager

Role OverviewIn this RN Field Clinical Care Manager role, you will be an essential element of an Integrated Care Model by relaying the pertinent information about member needs and advocating for the best possible care, ensuring they have the right services to meet their needs. Travel is expected throughout the advertised boroughs.
Primary Responsibilities- Assess, plan and implement individualized care strategies targeting the most appropriate, least restrictive level of care.
- Perform the NYS UAS Assessment in the member’s home at least twice per year and as needed.
- Identify and initiate referrals for social‑service programs, including financial, psychosocial, community and state supportive services.
- Manage the care plan throughout the continuum of care as a single point of contact.
- Communicate with all stakeholders the required health‑related information to ensure quality coordinated care and services are expeditiously provided to all members.
- Advocate for patients and families to ensure their needs and choices are fully represented and supported by the health‑care team.
- Current, unrestricted RN license in the state of New York.
- 2+ years of relevant clinical work experience.
- 1+ year of community case‑management experience coordinating care for individuals with complex needs.
- Experience in long‑term care, home health, hospice, public health or assisted living.
- Proficiency with MS Word, Excel and Outlook.
- New York state issued ability to obtain one prior to hire.
- Residence in New York state.
- Ability to travel in the assigned region to visit Medicaid members in their homes or other settings, including community centers, hospitals or providers’ offices.
- Behavioral‑health or clinical degree.
- Experience with electronic charting.
- Experience arranging community resources.
- Field‑based work experience.
- Background in managing populations with complex medical or behavioral needs.
- Proficiency in use of UASNY.
Pay for this role ranges from $40.00 to $54.00 per hour based on full‑time employment and includes a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (subject to eligibility). A sign‑on bonus of $7,500 is available for external candidates only; internal candidates are not eligible.
Remote Work PolicyAll employees working remotely will be required to adhere to United Health Group's Telecommuter Policy. If you are located in New York state, you will have the flexibility to work remotely as you take on some tough challenges, subject to the policy.
