Hospice & Palliative Care Buffalo is seeking a part-time Licensed Practical Nurse (LPN) Coordinator in the Admissions Department.
You will coordinate and implement the patient plan of care, handle calls from patients and caregivers, collaborate with RNs and the Team Manager, schedule visits, document data on time, communicate health status to the team, and assist with administrative duties as needed. New York LPN licensure is required.
