Salary

Earn between $29–$32 with a .75 shift differential.

Duties
:
- Collect required information from new Residents to be admitted
- Record health details of Residents; including vitals & temperature
- Administer medications and injections to Residents as needed
- Treat and dress wounds and bedsores as needed
- May be required to supervise Certified Nursing Assistants (CNAs)
- Help Residents get dressed & take care of personal hygiene
- Monitor Residents’ food and liquid intake and output
Requirements
:
- Must be able to work as a team member
- Valid NY State LPN license
- In good standing with State Registry
Location
:
Buffalo, New York.
We are seeking a dedicated full‑time (37.5 hours per week) Behavioral Health LPN. This position will cover services at several of our locations throughout the WNY area.

Duties
:
- Performing medical screenings and evaluations for the individuals we serve
- Medical injections
- Laboratory testing
- Phlebotomy experience required
- Obtaining prior authorizations as needed
- Facilitation of Medical Education Groups
- Assisting in Med Clinics
Requirements
:
- Graduated from an accredited college of Nursing and 2+ years experience in a similar position
- Salary starting at $21.94 - $27.42 per hour depending on education, credential and experience
- Up to $2,500 in sign on bonus

Responsibilities
:
- Assist in coordination and implementation of the patient plan of care
- Handles phone calls from the patient, primary care giver, or designee regarding any questions or concerns
- Collaborates with the interdisciplinary teams, RNs, or Team Manager
- Coordinates and schedules patient visits
- Provide timely documentation/data entry per Hospice time frames and deadlines
- Communicates current health status and needs of patients to appropriate team professionals
- Assists the team and Team Manager with other administrative functions, as necessary
Qualifications
:
Education/Certification:
Graduate of an accredited School of Practical Nursing with current LPN licensure in the State of New York.
Experience:
1 year of nursing practice as a Licensed Practical Nurse preferred.
