What You'll Do
- Respond to after-hours calls and urgent patient needs throughout the service area.
- Complete emergency hospice visits to assess changes in condition and provide expert symptom management.
- Deliver pain management, medication administration, wound care, and other comfort-focused nursing interventions.
- Provide compassionate telephone support and education to patients, families, and caregivers.
- Coordinate closely with physicians and the interdisciplinary hospice team to ensure continuity of care.
- Complete accurate and timely EMR documentation.
What You'll Bring
- Active Minnesota Registered Nurse (RN) license.
- Previous hospice nursing experience strongly preferred.
- Experience in home health, oncology, ICU, palliative care, or other high-acuity settings is highly valued.
- Excellent assessment, critical thinking, and independent decision-making skills.
- Outstanding communication and crisis management abilities.
- Ability to work evenings, overnight hours, weekends, and holidays as part of a 7 On / 7 Off rotation.
