Psychiatric Nurse Practitioner is responsible for providing psychiatric medication services to patients, many of whom experience co-occurring mental health and substance use-related concerns. This position collaborates with and supports the multidisciplinary mental health team to ensure a patient-focused, harm reduction, and trauma-informed approach to care.
As part of the essential functions of this role, the Psychiatric Nurse Practitioner:
- Screens patients for common psychiatric, substance use, and/or medical co-morbidities and evaluates co-existing conditions and the implications on the patient care/needs
- Ensures the timely completion of admission assessments, required psychiatric assessments, psychiatric, substance use, and medical histories and follow-ups, lab testing, as required by state and federal regulations, accreditation standards, and sound medical practice
- Prescribes and titrates the appropriate dosage of medications
- Writes medical orders based on the individual needs of the patient
- Works within an integrated treatment model coordinating treatment with both internal and external providers as indicated
- Serves as a role model for positive, warm, welcoming, productive, and cooperative relationships with staff and clients
- Adheres to Office of Mental Health (OMH) regulations and Evergreen Health policies and procedures, including regulatory time frames on clinical documentation, using an electronic medical record system
- Collaborates with and supports team members to ensure a patient-focused approach to care
- Attends and participates in case conference, center meetings, and program initiatives
Qualified Candidate will be a graduate of an accredited Psychiatric Nurse Practitioner training program, be a certified Psychiatric Nurse Practitioner, and be licensed to practice in New York State; candidate must have current BLS/CPR certifications, and experience utilizing EMR software (Medent preferred);
Two years of direct care practice experience as a PNP preferred; ability to travel between Evergreen locations is required; awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential
Job Type: Full time; schedule is negotiable between 10 hour shifts or 8 hour shifts
Required Education, license/certification: graduate of an accredited Psychiatric Nurse Practitioner training program, licensed to practice in New York State; certified PNP, with current BLS/CPR certifications
Experience:Two years direct care practice experience as a PNP preferred
