Skilled Nurse Practitioner with 1+ years of nursing home experience needed to provide monthly primary care services between two long-term care facilities in Buffalo, NY.
Compensation: $85 per hour
Benefits: Travel reimbursement + overnight stay as required
Schedule: 10 hours per week, flexible scheduling
Location: Two long-term care facilities in Buffalo, NY
Key Responsibilities- Primary Care Management
- Conduct monthly primary care visits as pertain to the CCM and CoCM programs. - Regulatory Documentation
- Maintain accurate, compliant clinical documentation for both primary care and wound care services.
- Active Nurse Practitioner license in New York with Board Certification.
- 1+ year of nursing home or long-term care experience.
- Strong clinical judgment and comfort working independently.
