Summary:
Join Our Team! We are seeking a compassionate, patient-focused, and highly organized Oncology Nurse Coordinator to serve as a primary clinical contact for patients and caregivers throughout their healthcare journey. This role is ideal for an experienced nurse who excels in care coordination, patient advocacy, communication, and delivering an exceptional patient experience. The Oncology Nurse Coordinator serves as the primary clinical contact for patients and caregivers, offering personalized support throughout the healthcare journey.
Includes:
- Coordinates multidisciplinary care and ensures efficient, effective communication among providers.
- Utilizes the nursing process to assess, develop, implement, and evaluate individualized plans of care.
- Acts as the central point of contact for patient and caregiver inquiries related to care plans.
- Proactively anticipates patient needs to ensure timely navigation.
- Assess learning needs and provides education to promote adherence to established care plans.
- Enhances the patient experience by delivering exceptional customer service and promptly addressing concerns.
- Performs all responsibilities aligned with the Nurse II role, with additional emphasis on care coordination, communication, and patient advocacy.
- Interacts with patients via phone, virtual visits, and the patient portal to ensure care is well coordinated.
- Starting hourly rate is $51.65 which includes a comprehensive benefits package.
Required Education and Experience License Requirement License to practice as a Registered Nurse in the State of New York. Certification in Basic Life Support (BLS) or eligibility for certification.
Education and ExperienceThe preferred candidate will have prior sarcoma and coordinator experience in an ambulatory setting.
