All Metro Health Care - Buffalo is seeking an experienced Per Diem RN Supervisor to support our home care clients and caregivers throughout Buffalo and surrounding communities.

In this role, you will have the opportunity to positively impact the lives of patients and families while supporting the success and development of our caregivers in the field.

• Supervisor:$35
  /Post Hospital Visit: $60
• Reassessment Visit: $75
• Initial Admission: $90 weekday / $125 weekend
• Mileage reimbursement provided at the current federal rate

• Weekday evening visits
• Weekend coverage
• Holiday availability
• Occasional after-hours admissions or supervisory visits

This is a flexible per diem position with variable scheduling needs based on patient care demands, admissions, reassessments, and staffing coverage.

Candidates with broader scheduling flexibility are strongly preferred.

• Conduct initial assessments, reassessments, and supervisory home visits
• Evaluate, educate, and support caregivers on assigned cases
• Provide clinical orientation and participate in field staff training
• Coordinate services based on patient needs
• Collaborate with physicians and care teams to develop and update plans of care
• Counsel patients and caregivers regarding nursing and care needs
• Complete accurate clinical documentation and progress notes
• Monitor records for accuracy, completeness, and compliance
• Identify and report changes in patient condition

• Active NYS Registered Nurse (RN) License
• CPR Certified
• 1 yearof
  
  Home Careexperiencerequired
• 1 year of supervisory or management experience required
• Valid NYS Driver’s License, insurance, registration, and reliable car
• Experience completing UAS assessments
• Familiarity with HHA Exchange (HHAx) or similar home care EMR/documentation systems
• Managed care, MLTC, or community health experience preferred
