Summary

The Advanced Practice Nurse practices in Endocrinology Service to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice - consultation - and management. The Endocrinology NP is responsible for managing and providing comprehensive healthcare to patients with endocrine disorders.

• VA Buffalo Healthcare System is seeking an enthusiastic Advanced Practice Nurse to join its Endocrinology staff
• The successful candidate will provide comprehensive care to include the diagnosis - treatment - and management of acute and chronic endocrine conditions such as diabetes - thyroid disorders - osteoporosis - and other hormonal imbalances
• The NP collaborates with a multidisciplinary team to deliver high-quality patient care
• DUTIES MAY INCLUDE HOWEVER ARE NOT LIMITED TO THE FOLLOWING:
  Independently manages a panel of patients with acute and chronic health conditions in inpatient and outpatient settings - with an emphasis on health promotion and disease prevention using evidence-based practice
• Develops and implements an individualized plan to achieve optimal population health and disease management
• Demonstrates leadership and works collaboratively with the interdisciplinary team
• Orders and interprets labs - tests - and diagnostic studies
• Prescribes pharmacologic and non-pharmacologic interventions
• Initiates referrals and consults to specialty services
• Provides education to patients and their families
• Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes

• Performs comprehensive history - physical exams - and diagnostic evaluations for endocrine patients across the age and complexity spectrum
• Provides patient and family education regarding endocrine disease processes - medications - lifestyle interventions - and long-term management
• Initiates referrals to appropriate subspecialties and coordinates multidisciplinary care
• Documents all encounters in the electronic medical record (CPRS) in a timely and accurate manner consistent with VHA requirements
• Participates in telehealth modalities - including Clinical Video Telehealth (CVT) and VA Video Connect (VVC)
• Incorporates research - evidence-based practice - established professional - community - or regulatory standards - and clinical practice guidelines to validate or change clinical practice
• Supports dissemination of evidence-based practice - i.e.
  
  - research - quality improvement monitors - best practices - and performance improvement results
• Identifies and evaluates clinical/process opportunities and trends for improvement and takes appropriate action for resolution
• Works effectively as a leader in planning and facilitating change to improve outcomes
• Collaborates with stakeholders to improve patient care - staff development - and performance measures
• Leads a patient care team
• Leads within a High Reliability Organization (HRO) framework
• Participates in identifying - analyzing - and resolving care problems
• Maintains effective collegial relationships with the interdisciplinary team - exercising sensitivity - active listening - and communication skills
• Provides the customers with consistent information according to established policies and procedures
• Protects printed and electronic files containing sensitive data in accordance with the provisions of the Privacy Act of 1974 and other applicable laws - federal regulations - VA statutes and policy - and VHA guidelines
• Preferred Experience:
  
  1-2 years of clinical experience in endocrinology care - with experience evaluating and managing common endocrine disorders including diabetes - thyroid disease - and osteoporosis
