PositionSummary
The Rapid Rehousing Case Manager works to support domestic violence victims and their dependent children in obtaining and maintaining permanent housing through housing search assistance, rental assistance, and case management assistance, using a trauma‑informed, empowerment‑based, housing first model of service provision.
Major Responsibilities/Activities- Complete intake procedure with clients, including all required paperwork and sufficient and acceptable documentation of Category 1 or 4 Homelessness, as defined by HUD.
- Assist clients in identifying and procuring permanent housing within 30 days of enrollment in the program.
- Ensure permanent housing identified meets Housing Quality and Rent Reasonableness Standards.
- Provide ongoing case management to clients to address barriers to maintaining permanent housing, individualized to the client’s identified needs, utilizing the progressive engagement model.
- Conduct required monthly home visits for all assigned client households.
- Complete referrals for clients to community services and resources to meet ongoing identified needs.
- Facilitate tenant/landlord mediation and dispute resolution as appropriate.
- Maintain accurate and up-to-date client files, statistics, case notes, and summary reports as required, including reports for funding sources as needed.
- Maintain accurate and up-to-date records of program funds utilized by clients enrolled in the program for housing costs, in conjunction with supervisor and Business & Finance.
- Attend Haven House staff meetings, CAP team meetings, trainings and regularly scheduled supervision.
- Participate in program audits and site visits, as needed.
- Provide occasional coverage in the shelter, if needed.
- Engage in staff development to ensure continued professional growth.
- Model and practice sensitivity, fair treatment and acceptance of diversity in all interpersonal interactions.
- Promote the empowerment of women as defined by the Social Work profession and Domestic Violence Services best practices, adhering to the principles of non‑violence and independent decision making.
- Participate and contribute to program Quality Improvement process.
- Perform other related duties as required.
- Bachelor of Social Work degree (BSW) or equivalent education and experience with at least two (2) years paid professional experience in housing services.
- Professional rapid rehousing program experience preferred.
- Bi‑lingual skills (Spanish-English) preferred.
- Professional domestic violence experience preferred.
- NYS driver's license and continued automobile insurance required.
Pay Rate of $26.58 per hour
