We are looking for caring and passionate Prevention Family Specialists for our Home Run program in Niagara County and Stepping Stones Program in Erie County.
What does a Prevention Family Specialist do?The Prevention Family Specialist goal is to preserve the family unit by providing supportive casework counseling in the home and empowering parents with tools and resources to ensure children’s safety, well‑being and permanency while providing advocacy to ensure quality services.
What is the Home Run Program?A school‑based prevention program designed to avert out‑of‑home placements of at‑risk children due to school‑related issues and difficulties with family functioning.
What is the Stepping Stones Program?An alternative to detention program which offers intensive crisis intervention. The program’s goal is to reduce the county’s need to place youth into detention facilities.
Job Responsibilities- Conduct assessment of family’s strengths and needs; assess problem areas and utilize various treatment modalities and resources to effect growth and change.
- Participate in development, implementation and ongoing evaluation of comprehensive treatment and discharge plan.
- Develop alternative placement or permanency resources for youth that may not return to their families.
- Complete a comprehensive discharge summary at least eight weeks prior to discharge.
- Deliver effective case management services, including ensuring counseling sessions are scheduled as needed and children receive required medical and dental examinations.
- Track and monitor services provided, complete all documentation in a timely and comprehensive manner.
- Ensure compliance with confidentiality standards, parental consent and necessary release of information.
- Conduct casework contacts with families, at least twice a month, in their homes and community settings to focus on treatment goals.
- Engage youth and family by developing an effective and caring relationship.
- Ensure children are maintained in a safe, secure environment.
- Develop and maintain relationships with community schools to ensure the best educational experience for each child and with community resource agencies.
- Develop and maintain partnerships with courts, probation, and local law enforcement.
- Minimum Bachelor’s degree in human services or a related field.
- At least 1 year of experience working with at‑risk youth and families and case management.
- Ability to work a flexible schedule with variable hours including day, evening, and subject to call hours.
- Subject to call:
Must be on call in collaboration with a Program Coordinator, with possible evening/weekend field work. On‑call is in rotation with the program Family Specialists. - Valid driver’s license with an acceptable driving history to the agency, and a reliable vehicle. Travel required.
- Ability to coordinate and plan travel for visits and appointments efficiently.
- Ability to work with an economically and culturally diverse population.
- Skills:
Ability to handle crisis intervention in a solution‑focused manner, and strong communication skills including written, verbal and listening skills. - Bilingual is a plus.
