Job DescriptionTo provide housing case management services and support to adult individuals who are homeless, or at risk of being homeless, with mental health, chronic health, substance use & co-occurring issues with the goal of obtaining and maintaining safe, affordable housing and residential stability in the community. Some participants may be being releasing from Correctional Facilities and/or from New York State Psychiatric Centers into the community.
EssentialFunctions
- Knowledge and demonstration of agency core values in day-to-day activities
- Work with individuals, in locating safe and affordable housing in an area of their choice
- Promotes and supports individual movement through program phases, including assisting customer to develop goals, and target activities leading to sustainable housing and successful graduation.
- Purchase individual household furnishings for participant residence
- Assist with obtaining benefits if needed, collect income, diagnostic and complete a rental stipend to determine voucher per month
- Maintain open communication with all treatment and service providers, families, landlords and any other supports or networks the individual is working within the community.
- Assist in outreach and location of the individuals referred
- Responsible for recordkeeping (progress notes) in the Millennium Database and other required paperwork such as service planning, roster maintenance, rental stipends, income and homeless verifications, and any other duties asked by supervisor of supportive housing & meets documentation timeliness standards, as required by the agency.
- Must have own transportation.
- There will be an expectation to see individuals in person twice each month, preferably in their home(s). Visit frequency is dependent on client need above that expectation.
- Attend team and community-based team meetings in the community
- Other duties as assigned
- Proficient in the use of computer software such as MSWord, Excel, Power Point, Desktop Publishing, Outlook, etc.
- Knowledge of Community Health Systems.
- Associate Degree with 1 years’ qualifying experience
- High School Diploma with 2 years’ qualifying experience
Qualifying Experience means-verifiable full or part time experience working with the following populations: mental health, substance use, chronic illness and/or co-occurring disorders.
Must possess a valid drivers license with a satisfactory driving record, and possess a personal vehicle for job requirement
Compensation$19.00-$22.26 with a $2,500 Sign on Bonus
