Critical Time Intervention (CTI) Case Manager
Help People Navigate Critical Transitions—and Build Stability That Lasts Endeavor Health Services is growing, and we're looking for a full-time Critical Time Intervention (CTI) Case Manager to support individuals during some of the most pivotal moments in their lives—such as transitioning from hospitalization or higher levels of care back into the community. In this role, you'll provide focused, time-limited support that helps people secure housing, access services, and build strong community connections.
You'll work as part of a collaborative team to ensure each person has the tools and support they need to achieve long-term stability and independence. $5,000 Sign-On Bonus
What You'll Do
- Conduct comprehensive assessments of client needs, strengths, and goals
- Develop individualized, short-term support plans focused on stability and transition
- Coordinate transitions from hospitals or other settings into the community
- Collaborate with healthcare providers, social services, and community organizations
- Connect people to resources including housing, employment, and healthcare
- Build strong, trusting relationships to encourage engagement in services
- Monitor progress and adjust plans to support successful outcomes
- Advocate for client needs across systems and services
- Maintain accurate, timely, and compliant documentation
- Participate in team meetings, training, and ongoing professional development
- Identify and address barriers to long-term stability
- Provide education and support to the people we serve and families
What We're Looking For
- Bachelor's degree in social work, psychology, or related field
- 2+ years of case management or related experience
- Valid driver's license and reliable transportation
- Flexibility for occasional evenings or weekends as needed
- Working knowledge of Microsoft Office
Compensation & Benefits
- Salary: $24.68–$27.67/hour (based on experience and credentials)
- $5,000 sign-on bonus
