SLP Opportunity At Trans Perfect Health

Join the Trans Perfect Health family at Holbrook Sugar Hill, a premier assisted living and memory care facility serving the active-aging community in the desirable Sugar Hill City Center. Located at 5135 West Broad Street NE, Sugar Hill, GA 30518.

Currently, we are seeking an SLP that (A) loves the aging population and (B) has at least 8-10 weekday hours of availability between 9AM and 3PM. We will allow the hours to be broken up at the SLP's convenience.

Key Responsibilities
  • Conduct screenings, evaluations, assessments, and treatments in accordance with state practice acts, plan of care, and company policies
  • Help optimize functional self-care, independence, and quality of life for members of the aging population
  • Accurately complete all required documentation in a timely manner
  • Participate in required orientations, staff meetings, and in-service education
  • In collaboration with the Director of Rehabilitation, develop robust therapy program
  • Provide clinical supervision to Therapy Assistants as indicated
  • Follow all infection control procedures and OBRA Resident Rights Guidelines
  • Demonstrate sound clinical judgment and effectively prioritize patient care
  • Support compliance with all state and federal regulations
  • Perform duties that may involve moderate to heavy physical demands
  • Carry out additional responsibilities as assigned
Qualifications
  • Current or eligible Occupational Therapy license in the state of practice
  • Graduate of an accredited occupational therapy program
  • ASHA accreditation or equivalent
