Zona Physical Therapy & Wellness is a locally owned outpatient healthcare practice serving Buford, Gwinnett County, and surrounding communities.

We provide individualized physical therapy services for patients recovering from pain, injury, surgery, neurological conditions, balance problems, pelvic floor dysfunction, workplace injuries, and motor vehicle accidents.

Our physical therapy services include:

• Orthopedic and musculoskeletal rehabilitation
• Back, neck, shoulder, hip, knee, foot, and ankle rehabilitation
• Post-surgical physical therapy
• Neurological rehabilitation
• Balance and fall-prevention therapy
• Women's health and pelvic floor physical therapy
• Workplace injury rehabilitation

Our ownership team is also exploring expansion into home health care and other healthcare services. The future home health operation may be developed as an independent brand, an affiliated company, or through a franchise relationship. The final name, structure, and launch timeline have not yet been determined.

We are seeking a Healthcare Outreach Liaison who can initially support the growth of Zona Physical Therapy and potentially expand into a broader liaison role as additional healthcare services are introduced.

Zona Physical Therapy & Wellness is seeking an outgoing, dependable, and relationship-focused Healthcare Outreach Liaison to represent our practice throughout the local healthcare community.

The Healthcare Outreach Liaison will develop and maintain relationships with physician offices, hospitals, medical practices, discharge planners, case managers, attorneys, employers, senior communities, and other potential referral partners.

The initial focus of this position will be increasing awareness of Zona Physical Therapy's services and strengthening our patient referral network.

As our ownership group expands into home health care or other healthcare services, the successful candidate may be offered additional hours and responsibilities supporting those new service lines. This could include representing a separately branded, affiliated, or franchised home health organization.

This opportunity is ideal for someone who enjoys meeting people, building professional relationships, working independently, and helping connect patients with appropriate healthcare services.

• Conduct in-person visits to physician offices, specialty practices, hospitals, urgent care centers, and other healthcare facilities.
• Introduce Zona Physical Therapy and explain our clinical services, specialty programs, and referral process.
• Build professional relationships with physicians, referral coordinators, office managers, nurses, case managers, discharge planners, and other healthcare professionals.
• Educate referral partners about the patient populations and conditions treated at Zona Physical Therapy.
• Deliver brochures, referral forms, service information, and other approved outreach materials.
• Follow up consistently with existing and prospective referral partners.
• Identify new referral and partnership opportunities throughout Gwinnett County and nearby communities.
