Your Health Organization in the Buford area is seeking a Registered Nurse to join our Visiting Nurse/Case Management team. The Nurse Case Manager will visit patients in homes and facilities, coordinate with providers, and initiate telemedicine visits as needed.
Travel is required daily with reliable transportation. This is a full-time, salary-based role with 8-hour shifts, Monday through Friday, 8am–5pm, serving Bogart, Loganville, Winder and surrounding areas.
