Community Outreach Executive (Bukit Panjang/Pasir Ris)
Support implementation of outreach strategies including home visits, befriending support, outreach activities and community screening.
Engage seniors, build rapport, and encourage participation in AAC programmes.
Provide information and refer seniors to appropriate health or social services and follow up with those who require additional support or engagement.
Ensure quality and effective delivery of AAC core functions and services, while achieving established participation targets.
Plan, coordinate, and implement programmes that meet seniors’ needs and interests, ensuring high‑quality delivery that supports their physical, cognitive, and social well‑being.
Mobilise and encourage seniors to participate in programmes, ensuring inclusive and meaningful engagement.
Monitor participation levels and promote inclusive participation among seniors.
Recruit, train, and support volunteers to enhance outreach and programme delivery.
Provide supervisory support to Community Outreach Associates (COA).
Monitor COA’s competency development by arranging and monitoring required training.
Assist in building a positive volunteer culture aligned with AAC goals.
4.Operations, Administration & Compliance
Support the Manager in ensuring compliance in day-to-day operations.
Track KPIs, report outcomes, and implement service enhancements.
Support administrative processes including handling of enquiries, maintaining of documentation and ensuring accurate and timely reporting.
Undertake any additional responsibilities assigned by the Manager, Active Ageing Centre (AAC) to support care integration and community engagement.
5.Others
Any other duties as assigned by the Manager.
Requirements:
A degree in any discipline, preferably in Business Administration, Healthcare Administration, or healthcare-related field.
Experience in community care, programme planning, volunteer management preferred.
Good communication and interpersonal skills.
Team player who is organized.
Eye for details and meticulous in planning of events from start to the end.
Able to multi-task and perform in a fast-paced environment.
Positive, can-do attitude, with interest to engage the seniors.
Proactive, adaptable, and collaborative.
Willing to work on occasional weekends / night-events.
Adaptable to changes.
Proficient in Microsoft Office and data presentation.
