The Nurse Practitioner will independently work within a Specialty Care Team, providing advanced clinical services focused on patient-centered care. This role involves comprehensive patient assessment, diagnosis, treatment planning, and ongoing management to promote health and well-being. The Nurse Practitioner will collaborate with healthcare professionals to coordinate care and maintain detailed clinical records, ensuring optimal patient outcomes.

Preferred Qualifications

• Bachelor's degree in Nursing
• Active Nurse Practitioner (NP) certification

• Skilled in medication management and patient education
• Experience with Electronic Health Records (EHR) systems
• Strong communication, critical thinking, and teamwork abilities
• Effective time management skills
