• Doctorate degree in Physical Therapy or a Bachelor’s degree in Physical Therapy with NPTE passed.
• Graduated from an accredited school.
• Minimum one year of experience.
• Current state license as a Registered Physical Therapist / PT or actively pursuing licensure.
• Current CPR certification.
• Additional client‑facility specific requirements as determined by the facility.

The Physical Therapist/PT is responsible for assessing referred patients requiring rehabilitation services, including direct patient care, evaluating medical condition, functional capabilities, limitations, and potential for rehabilitation. The PT will establish and administer a treatment program with specific goals, set realistic achievable goals, document patients’ condition, and educate patients and families. Responsibilities also include:

• Developing a plan of care for each physical therapy patient.
• Providing skilled physical therapy services/interventions per physician orders.
• Assessing patient needs, planning, evaluating and modifying care to meet therapy goals.
• Collaborating with all disciplines to plan and evaluate team goals.
