Day shift, 5x8 hours, from 08:00 to 16:30.

Typical weekend requirement: one full weekend per 4 weeks, subject to department needs. No block scheduling is offered.

• 2 years of acute medical Social Work experience, including roles in Social Work and discharge planning.
• First-time travelers must pass a screening process.
• No time-off restrictions greater than 7 days.

• Provide comprehensive assessments and interventions for patients and families.
• Facilitate discharge planning and coordinate services for continuity of care.
• Advocate for patients' needs and assist in navigating healthcare systems.
• Collaborate with healthcare teams to develop discharge plans.
• Maintain accurate documentation of services provided.
