Description

Care Manager RN at Providence Saint Joseph Medical Center in Burbank, CA. This is a full‑time position with 8‑hour day shifts.

Case Management is a collaborative practice that includes patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. It facilitates communication and care coordination along a continuum, using effective transitional care management and recognizing patients’ right to self‑determination, social determinants of health, and care complexities.

• Bachelor’s Degree in Nursing, preferred.
• All Registered Nurses without a Bachelor’s degree must obtain a Bachelor’s degree in Nursing or higher within three years of hire.
• Any collective bargaining contract specifying time frames for obtaining a Bachelor’s degree in nursing supersedes the above clause.
• Exceptions are considered on a case‑by‑case basis by the Director of Care Management in consultation with Human Resources.
• RNs employed prior to 8/2021 are encouraged to obtain a Bachelor’s degree or higher; this will not be a condition of employment.
• California Registered Nurse License upon hire.
• Two years of experience in a healthcare related field (Acute, Ambulatory, Post‑Acute, etc.).
• Two years of experience in Case Management (Care Coordination or Utilization Management) or successful completion of the Transitions in Practice (TIP) program. TIP candidates must have experience in the same type of nursing unit as the CM position.

• Master’s Degree in a healthcare related field.
• Accredited Case Manager (ACM) upon hire.
• Certified Case Manager (CCM) upon hire.

Hourly: $56.12 – $84.75. Additional compensation may be available for shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
