This is a travel/contract nursing position for a Registered Nurse specializing in Case Management at an acute care facility in Burbank, CA. The role involves Care Coordination and Discharge Planning in an acute medical setting. First-time travelers are eligible but must be screened prior to placement.
Key Responsibilities- Perform Care Coordination and Discharge Planning in an acute medical care environment
- Maintain a caseload ratio of 1:25 on day shift
- Work typically one full weekend per four weeks, subject to departmental need
- 2 years of acute medical Care Manager experience performing Care Coordination and Discharge Planning
- Ability to adhere to department staffing ratios (Days 1:25)
- Professional demeanor appropriate for business attire or scrubs environment
- California (CA) State RN License required (pending license accepted, but must be licensed upon start date)
- Minimum 2 years of acute medical Care Manager experience performing Care Coordination/Discharge Planning
