Job Summary

This position will report to the Program Director and is responsible for creating partnerships and linkages, identifying individuals in need of services, and developing a coordinated outreach plan for suicide prevention within the Veterans Program. The Community Outreach Case Manager will work a conventional work week that includes evening hours and may require weekend work and will collaborate with other organizations to improve referral pipelines and care within VOACC’s Suicide Prevention services.

Responsibilities
  • Coordinate outreach efforts for the Veterans Program, establish an outreach plan targeting the three categories of eligible participants, develop a schedule to visit the VA, shelters, and other places veteran families congregate, and maintain communication with the VA and community providers.
  • Screen potential program participants for eligibility using VA‑approved forms and verify information with applicable agencies, including veteran and income status.
  • Provide SSG Fox SPGP services as outlined in program guidelines:
  • Outreach to identify those at risk of suicide
  • Baseline Mental Health Screening for participants over 18 years of age
  • Education on suicide risk and prevention to families and communities
  • Provision of clinical services for emergency treatment
  • Case management services
  • Peer support services
  • VA benefits assistance for eligible individuals and their families
  • Assistance with obtaining and coordinating other federal, state, local, or eligible entity benefits
  • Assistance with emergent needs related to health care, daily living, personal financial planning, transportation, temporary income support, fiduciary/representative payee, legal services, and childcare
  • Nontraditional and innovative approaches and treatment practices as determined by VA
  • Other services necessary to improve mental health status and reduce suicide risk of eligible individuals and families as determined by VA
    • Provide or arrange support and wraparound services, including connections to all eligible VA and mainstream resources.
    • Communicate effectively under confidentiality rules with the service team to address and correct issues in services, such as living skills, employment, education, family, and socialization.
    • Participate in program and professional supervision, attend required staff and Project Management Team meetings and trainings, and contribute to team‑based collaborative planning.
    • Complete other duties as needed and requested.
    • Attend monthly community outreach meetings.
    • Assist neighbors experiencing homelessness with accessing resources and making referrals; continue engagement until shelter or permanent housing is obtained.
    • Maintain computerized client records, daily activity logs, mileage logs, and other directed reports.
    • May drive a van/shuttle, complete daily vehicle inspection, perform vehicle cleaning, and maintain all safety conditions.
    • Develop relationships with neighbors through frequent outreach visits to build trust and support participation in programs.
    • Research community resources that benefit veterans.
    • Advocate for veterans when they encounter barriers.
    • Integrate the program throughout existing departmental and regional programs and services, including homeless services, veteran’s services, behavioral health, etc.
    • Travel throughout the community and local sites.
    • Other duties as assigned.
    Qualifications
    • Bachelor’s degree in Human Services or related field required.
    • 3 to 5 years of experience in outreach, especially with Veterans or individuals with mental health, substance use, or IDD concerns.
    • Demonstrated ability to explain agency purpose, programs, and operations.
    • Knowledge of available community resources.
    • Self‑starter, mission‑driven and committed to helping people connect with services.
    • Excellent computer skills in Microsoft Office and related products.
    • Excellent written and verbal communication skills.
    • Proven ability to resolve conflicts.
    • Bilingual in any language is a plus.
    Physical Requirements
    • Safe operation of a motor vehicle to transport self, consumers, and program supplies as needed.
    • Physical ability to travel, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear, and perform basic light home maintenance activities and operate office equipment.
    • Operate office equipment requiring continuous or repetitive hand/arm movements.
    • Remain seated for extended periods.
