Heritage Home Health and Hospice in Idaho is seeking a Physical Therapist to join our patient‑centered care team. You will perform home visits, coordinate care with families and other health professionals, and ensure visits comply with agency, state, and federal policies.
The PT must be licensed in Idaho, with CPR (BLS) certification, and the ability to document using industry software. A valid driver’s license and automobile liability insurance are required as you travel to patients' homes.
