Position:
Wellness Nurse
Pay Range: $35.00-$45.00 per hour.

- Communicate with community healthcare professionals—physicians, discharge planners, nurses, and state agencies—on the care needs of all residents.
- Coordinate incidental medical, dental, vision, hearing, and podiatry care for residents.
- Perform nursing duties as directed and within the scope of the Nurse Practice Act, including medication administration and treatments ordered by a physician.
- Provide support to Medication Technicians to ensure competent delivery of medications.
- Assist with resident assessment tasks as directed by the Health Services Director.
- Conduct staff interviews and observe residents continuously to inform staff of changes in physical and emotional health; communicate information to physicians and other healthcare professionals.
- Complete thorough documentation in the resident chart per company policy and state regulations.
- Review and monitor staff use of the Shift Report daily; participate in the change‑of‑shift meeting to communicate pertinent resident care information.
- Coordinate prescription orders with doctors’ offices and pharmacy delivery of medication; oversee the central storage and delivery of medications.
- Communicate with residents and responsible parties regarding the community’s preferred pharmacy and incontinence supply programs.
- Licensed Vocational or Registered Nurse with a valid California nursing license in good standing.
- Prefer one year of experience working in a Residential Care Facility for the Elderly (RCFE).
- Proficient in operating a personal computer and MS Office programs (Word, Excel, etc.).
- Able to use standard office equipment, including fax machine, copier, and telephone.
- Knowledge of, or willingness to learn, assisted living and dementia care practices.
- Ability to work patiently with seniors and cognitively impaired individuals.
- Valid first aid and CPR certification.
- Must pass a criminal background check and health screening tests, including physical and TB tests.
- Oakmont Management Group may require vaccination, daily screening, surveillance testing, and PPE to prevent communicable diseases, per regulatory guidelines.
