The Wellness Nurse is a licensed nurse responsible for coordinating resident care and medication management under the medical direction of the residents’ attending physicians to ensure residents remain as independent as possible. The nurse works during the Health Services Director’s scheduled off-days, providing consistent coverage across the community seven days a week.
Responsibilities- Communicate with community healthcare professionals, including physicians, discharge planners, nurses, and state agencies about resident care needs.
- Coordinate incidental medical, dental, vision, hearing, and podiatry care for residents.
- Perform nursing duties per the Nurse Practice Act, administering medications and performing treatments as ordered by a physician.
- Support Medication Technicians to ensure competent delivery of medications.
- Assist with resident assessment tasks directed by the Health Services Director.
- Conduct staff interviews and observe residents continually to inform staff of changes in physical and emotional health; communicate relevant information to physicians and other healthcare professionals; alert the Executive Director and responsible party to concerns as needed.
- Complete thorough documentation in the resident chart on an ongoing basis according to company policy and state regulations.
- Review and monitor staff use of the Shift Report daily and participate in the change of shift meeting to communicate pertinent resident care needs.
- Coordinate prescription orders with doctors’ offices and pharmacy delivery; oversee central storage and delivery of medications.
- Communicate with residents and responsible parties regarding the community’s preferred pharmacy and incontinence supply programs.
- Must be a Licensed Vocational or Registered Nurse holding a valid California nursing license in good standing.
- Prefer one (1) year of experience working in a Residential Care Facility for the Elderly (RCFE).
- Proficient in operating a personal computer and MS Office programs (Word, Excel, etc.).
- Proficient with standard office equipment, including fax machine, copier, telephone.
- Knowledge of assisted living and dementia care or aptitude to learn.
- Ability to interact patiently with seniors and cognitively impaired individuals.
- Valid first aid and CPR certification.
- Must pass a criminal background check and health screening tests, including physical and TB tests.
- Participate in COVID-19 preventive measures, including vaccination, daily screening, surveillance testing, and wearing of face coverings and PPE as required by regulations.
