Responsibilities for Clinical Liaison:
- Build and maintain strong, professional relationships with medical professionals, hospitals, and other healthcare facilities and organizations to support effective care coordination and referrals.
- Promote the organization’s philosophy, mission, and objectives both internally and externally, serving as a trusted representative of our care standards and values.
- Attend and actively participate in facility meetings, calls, and in-services to support collaboration, education, and strong communication across care settings.
- Demonstrate honesty, integrity, and professionalism in all interactions, and ensure the respectful handling of patient information and hospital property.
- Maintain accurate, up-to-date referral-tracking logs to support efficient admissions and care coordination.
- Prepare and distribute timely end-of-day reports to department leaders to ensure clear communication and operational alignment.
- Collaborate closely with interdisciplinary teams to support smooth transitions of care and ensure resident needs are clearly communicated and addressed.
- Valid driver’s license required, as travel between facilities and partner organizations is essential.
- Current RN, LPN, or LSW licensure preferred.
- Associate’s degree in a related field required;
Bachelor’s degree preferred. - Strong communication and relationship-building skills with the ability to engage effectively with healthcare professionals and community partners.
- Ability to work collaboratively with multiple residents, patients, and care teams in a fast-paced healthcare environment.
- Strong organizational skills with attention to detail and accuracy in documentation and reporting.
