Position: Registered Nurse (RN) - Home Health Admission - $60+ per visit

Job Summary

BAYADA Home Health Care is seeking a Registered Nurse (RN) Home Health Admission for a nursing job in Burlington, Massachusetts. This is a full‑time, ongoing position focused on completing OASIS Start of Care (SOC) visits for adult and geriatric patients across northeastern Massachusetts.

Responsibilities
  • Perform OASIS Start of Care (SOC) visits for adult and geriatric patients in the home setting throughout northeastern Massachusetts.
  • Conduct comprehensive head‑to‑toe assessments and identify subtle changes in patient condition.
  • Triage and manage urgent clinical situations independently.
  • Perform medication reconciliation, teaching, and safety monitoring.
  • Complete wound assessments, provide wound care, and document appropriately.
  • Utilize acute‑ and post‑acute care experience (e.g., CHF, COPD, wound care, IV therapy, post‑surgical care) to deliver quality care.
  • Maintain efficient and accurate point‑of‑care documentation using required technology.
  • Manage a dynamic visit schedule, prioritize urgent visits, and adjust to changing patient needs.
Qualifications
  • Current nursing license in good standing in Massachusetts.
  • Graduation from an accredited and approved nursing program, verified by transcript or diploma.
  • At least one year of recent hands‑on medical experience with an adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med‑surg, sub‑acute care, or rehab units.
  • Ability to work independently while knowing when to seek escalation.
  • Comfortable making real‑time clinical decisions without immediate on‑site support.
  • Strong problem‑solving skills in unpredictable home environments.
  • Experience handling dynamic visit schedules and prioritizing urgent care.
  • Attention to detail while meeting productivity expectations.
  • Cultural sensitivity and respect for patient autonomy.
  • Adaptability to changing patient needs and schedules.
  • Proficiency with Medicare coverage requirements and OASIS documentation.
  • Strong point‑of‑care documentation skills with relevant technology.
