Responsibilities

Primarily provide outpatient rehabilitation services in an in‑home setting for the older adult population, including private residences and independent living facilities.

Evaluate, develop, and deliver individualized physical therapy programs to facilitate rehabilitation.

Identify the need for and make referrals to other disciplines to address comprehensive patient needs.

Design and implement individualized therapy programs to restore, reinforce, and enhance performance.

Instruct, educate, and train patients and caregivers in skills essential for promoting independence and productivity.

Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as required.

Maintain adherence to department and patient treatment schedules, timely clinical and billing documentation, and work‑related meetings.

Attend and contribute to patient care and staffing conferences and related meetings.

Consult with the rehabilitation team to select appropriate therapeutic regimens.

Adapt program and/or plan of care according to individual patient needs.

Promote a safe environment and enforce elimination of fire and safety hazards.

Order supplies and equipment as necessary; clean, maintain, and report equipment needs.

Deliver an initial screen, examination, and evaluation, establish diagnosis, identify impairments, set treatment goals, and implement the treatment plan.

Provide therapeutic techniques, including:

Therapeutic exercise (aerobic conditioning, balance, agility, postural stabilization, flexibility, gait training, neuromotor development, relaxation, strength, power, endurance)
Functional training
Manual therapy
Airway clearance techniques
Integumentary repair and protection
Electrotherapeutic, physical agent, and mechanical modalities
Health and wellness programming
Education on chronic disease management
Falls assessment and interventions to reduce fall risk
Case management in the home and community

Design, fabricate, fit, or apply adaptive devices and train patients in their use (orthotics and prosthetics).

Select activities suited to the individual's physical, mental, and cognitive capacity.

Instruct patients in mobility skills and evaluate progress, attitudes, and behavior related to rehabilitative potential.

Perform routine re‑examination and modify the plan of treatment as needed.

Incorporate health and wellness programming into treatment plans.

Conduct home and community mobility assessments and recommend safe mobility solutions.

Perform additional related duties as required.

Qualifications

Valid Physical Therapy license in the state(s) of practice or proof of eligibility.

Master's degree in Physical Therapy.

Master's degree in a related field plus a Bachelor's in Physical Therapy.

Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience.

Degree earned outside the U.S. deemed equivalent to a U.S. PT degree.

Valid driver's license or reliable, independent transportation to patients' residences.

Posted Salary Range

USD $46.00 - $62.00 /Hr.
