• Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with activities of daily living (ADLs)
• Monitor and report changes in resident condition to the nursing team, ensuring timely interventions and effective communication
• Collaborate with interdisciplinary teams to develop and implement individualized care plans, promoting resident autonomy and dignity
• Participate in quality improvement initiatives, incorporating evidence-based practices to optimize patient outcomes
• Maintain accurate and detailed records of patient care, adhering to regulatory requirements and organizational standards
• Demonstrate a strong commitment to patient-centered care, prioritizing resident needs and preferences
