Job SummaryAuthora Care Collective is seeking a Referral Intake Specialist. This part‑time hybrid position is scheduled Tuesday and Wednesday 10:00am – 7:00pm (16 hours per week).
The Referral Intake Specialist assists prospective patients and families by explaining the organization’s services, insurance information, admission criteria, and related information. The role initiates the referral process and provides clerical support for admissions and patient care services, including handling incoming and outgoing email and data entry.
Key Responsibilities- Answer the referral line and take referrals.
- Enter referral data into the database.
- Provide hospice and palliative phone consults, explaining services, reimbursement sources, and transfer of services.
- Support walk‑in referrals, procure physician consent to be attending, and obtain certifications of terminal illness.
- Request and obtain patient medical records and scans into the EMR.
- Collect data and update communication in the EMR.
- Notify the appropriate team with new referral information and patient location.
- Ensure the team has proper paperwork prior to scheduling assessment visits.
- Schedule RN visits for patients.
- Secure necessary signatures and paperwork.
- Coordinate smooth transition of services.
- Make follow‑up calls as needed to ensure complete information for admission.
- Work closely with the Hospital Liaison Team, Community Liaison Team, and other Referral teams.
- Maintain intake efficiency for rapid turnaround of referrals.
- Associate degree or equivalent work experience.
- Minimum one year of office experience using medical secretary skills.
- Successful completion of an administrative program preferred.
- Computer/data processing training or experience preferred.
- Healthcare experience preferred.
- Knowledge of medical terminology preferred.
- Typing speed of 50 words per minute preferred.
Other
:
Valid state‑issued driver’s license required. Must carry automobile liability insurance at limits required by agency and have own transportation.
