Brief DescriptionAuthora Care Collective is currently seeking a Referral Center Intake Specialist. This position is Full-time hybrid, Monday - Friday 9:00am - 6:00pm with rotating weekends and holidays.
Assists with prospective patients and families by explaining the organizations services and programs, insurance information, admission criteria and related information. The Referral Center Intake Specialist will begin the referral process, coordinating and providing clerical support for admissions and patient care services. Assists with incoming and outgoing email and data entry.
Key Responsibilities- Assists Answering Referral Line and Taking Referrals.
- Assists in answering Referral Center phone line, taking referrals and data entry of referrals into database.
- Provides hospice and palliative phone consults, explaining what services Authora Care provides, reimbursement sources, and transfer of services.
- Assists with providing support for walk-in referrals. Procures physician's consent to be attending and certifications of terminal illness.
- Requests and obtains patient medical records and scans into the EMR.
- Collects data and updates communication in EMR.
- Notifies appropriate team with new referral information and patient location.
- Ensures that team has proper paperwork prior to scheduling assessment visits.
- Schedules RN for patient visits appropriately.
- Ensures all paperwork for signatures are in place.
- Coordinates smooth transition of services.
- Makes follow-up telephone calls as needed or directed to ensure complete information is received and given as needed to admit a referral.
- Works closely with Hospital Liaison Team, Community Liaison Team and other Referral.
- Intake members for rapid turn-around of referrals.

Education/
Experience:
- Associate degree or equivalent work experience.
- Minimum of one-year office experience in a position using medical secretary skills.
- Successful completion of administrative program preferred.
- Computer/data processing training/experience preferred.
- Healthcare experience preferred.
- Knowledge of medical terminology preferred.
- Ability to type 50 wpm preferred.
Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
