Overview

Amedisys is recruiting a Registered Nurse Case Manager to support patient care and coordination. The position is full‑time and offers a sign‑on bonus of $15,000, with an RN license required for employment.

• $71,713.20 – $89,641.50

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care
• Makes referrals to other disciplines as indicated by the patient’s needs or document the rationale for not doing so
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including self‑care techniques
• Supervise LPNs and HHAs
• Complete documentation timely and accurately
• Communicate patient progress regularly to the clinical manager and care team
• Plan and provide staff education
• Perform on‑call responsibilities and provide on‑call services to patients/families as assigned
• Participate in clinical development and continuing education programs
• Other duties as assigned

• One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year, approval by VP Clinical is required
• Current RN license specific to the state(s) of assignment
• Current CPR certification
• Valid driver’s license, reliable transportation, and liability insurance
• If less than 6 months of clinical experience as an RN, participation in the RN Intern program is required
