Monday through Friday, 8:00am – 5:00pm. After‑hours availability on a rotational basis for urgent home visits between 8:00am–5:00pm on weekends.

Working under the direct supervision of the Clinical Services Director, the RN Care Manager provides direct and indirect nursing care to assigned patients by utilizing critical thinking skills to assist in the management of chronic diseases, clinical problem solving, and decision making in collaboration with the IDT. The RN Care Manager applies advanced nursing knowledge and skills relative to NCBON scope of practice, competency and experience independently.
Responsibilities include participating in call rotation, performing home visits, end‑of‑life/palliative care (aka Pathways), and nursing care per nursing protocol. The RN Care Manager completes professional performance requirements annually.
Graduate of an accredited school of nursing.
Preferred Qualifications- Has practiced nursing in the last 3 years.
- Preference for experience in home care or long‑term care settings, and ideally PACE experience.
- Two years of acute care management experience preferred and/or 1 year of community‑based nursing experience.
- Must possess knowledge and skills necessary to treat patients.
- Meet a standardized set of competencies for the position established by PHSC and approved by CMS before working independently.
- Excellent interpersonal, oral and written communication skills.
RN currently licensed in NC; present credentials demonstrating ability to meet performance criteria for the RN position;
Valid NC driver’s license.
Be medically cleared for communicable diseases and have all immunizations up‑to‑date prior to beginning employment.
Pay Range$71,088 - $95,536 (commensurate with years of experience)
