Job DetailsFull-time (32 hours per week / 4 days). CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
Job SummaryManage the overall plan of care for a caseload of hospice patients in collaboration with other members of the interdisciplinary group (IDG). Assess and identify patient/caregiver problems, establish nursing goals, and interventions. Work with physicians to establish the medical plan of care for the patient.
Establish and maintain a therapeutic relationship with patients and caregivers, including providing emotional support and teaching. Work closely with the interdisciplinary group, communicating effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health‑care professionals. Document the outcome of care and provide dietary counseling for patient care needs as indicated. Assume on‑call responsibilities as scheduled.
Requirements- Graduate of an accredited School of Nursing
- Must be currently licensed as an RN in North Carolina and in good standing with NCBON.
- Experience in an acute, hospice, home health setting, or long‑term care setting is a plus.
- CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
- Valid state‑issued driver’s license required. Must carry automobile liability insurance at limits required by the agency. Must have own transportation.
