Home Health Registered Nurse (RN) - Burlington, North Carolina

An exciting opportunity is available for an experienced and compassionate Registered Nurse with expertise in home health. This contract role is essential in delivering high-quality, individualized nursing care to patients within their homes, ensuring both clinical excellence and patient comfort.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Complete OASIS-based comprehensive patient assessments and establish tailored care plans.
• Deliver skilled nursing interventions including wound care, IV therapy, chronic disease management, and medication administration.
• Track patient progress and promptly communicate any status changes to physicians and the care team.
• Provide patient and family education focused on disease management, medications, and preventive health.
• Maintain accurate, timely notes using Home Care Home Base (HCHB) electronic documentation.
• Collaborate closely with multidisciplinary professionals to ensure cohesive care delivery.
• Attend regular case conferences, team meetings, and engage in continuing education.
• Adhere strictly to regulatory and compliance standards for home health services.

Qualifications:

• Active RN license (in good standing) in the state of practice.
• Minimum of 1 year of recent home health field experience.
• Proficiency in Home Care Home Base (HCHB) and completion of OASIS assessments.
• Demonstrated clinical assessment, critical thinking, and time management skills.
• Reliable transportation, valid drivers license, and current auto insurance.
• Ability to independently manage a dynamic, field-based caseload (35 visits daily).
• Strong communication and organizational abilities.
• BSN preferred and prior experience managing a caseload of 2532 points per week highly desirable.
• Familiarity with Medicare and home health compliance standards is a plus.
