RN - Case Management

Day shift, 5x8 hours (07:00 AM - 03:30 PM), Monday - Friday

We are seeking an experienced Registered Nurse (RN) specialized in Acute Care to join our team. This position requires a strong background in various specialties, including ICU, MS, PICU, SDU, PCU, IMC, and Obs. The ideal candidate will have previous charge experience and a commitment to quality patient care.

• Conduct thorough patient assessments and develop plan of care.
• Manage patient care in collaboration with healthcare teams.
• Perform discharge planning and needs assessments.
• Order durable medical equipment (DME) as needed.
• Adhere to regulatory requirements set by CMS.

• Minimum 2 years of experience in an acute care setting.
• Active RN license required.
• BLS certification required.
• Experience with EPIC electronic medical records system.
• Pass a compliance EKG exam (2 attempts allowed).
