Job Purpose

This position is currently grant-funded through pe r-member-per month payments tied to contracts between Medicaid, insurance companies, and local health departments. At this time, funding is secured through December 31, 2026. Join a mission-driven team making a real impact in Alamance County. We’re seeking a dedicated public health nurse to provide care management and program coordination for the CMHRP program. In this role, you’ll support individuals and families through clinic work, home visits, and community partnerships—helping improve health outcomes where it matters most.

You’ll use strong clinical judgment, public health knowledge, and care management skills as part of a multidisciplinary team. Work is guided by the NC Nurse Practice Act and evidence-based public health standards. This position is ideal for a nurse who values autonomy, meaningful relationship-building, and improving population health. As part of Public Health, you may also assist in emergency preparedness efforts such as vaccination clinics, shelters, or medication distribution.

Responsibilities

Provide comprehensive CMHRP Care Management services for pregnant Medicaid members who are at risk for poor birth outcomes. In this role, you will assess, plan, coordinate, and evaluate care while offering support to meet the medical, preventive, and social needs of pregnant and postpartum women. You will connect patients with medical, nutritional, educational, social, and emotional resources to promote healthy pregnancies and positive birth outcomes.

Responsibilities include clinic, community, physician office, and home visits to meet program requirements. You will serve as part of a multidisciplinary Care Management Team—providing leadership for medically complex patients and acting as a nursing resource for non-nurse team members. You will work toward program goals established by the State Medicaid program and prepaid health plans. Additional duties include using multiple electronic medical record systems, reviewing and reporting program data, assisting with staff onboarding and training, and supporting student learners.

This position also provides direct supervision for 3.5 multidisciplinary team members.

Qualifications

Graduation from an accredited school of professional nursing and one to two years of professional nursing experience and demonstration of leadership experience such as charge nurse, preceptor, committee chair, or direct supervision of staff.

Preferred Qualifications

Fluent in both Spanish and English both oral and written.

License And Certifications

Current licensure as a Registered Nurse in North Carolina by the North Carolina Board of Nursing and a valid North Carolina driver's license are required, or must be obtained within 60 days of relocating to North Carolina.
