Authora Care Collective seeks a Hospice RN Home Care Case Manager for the Burlington service area. The position is full-time (32 hours per week / 4 days) with CPR/BLS certification required.
The RN will manage care plans, collaborate with the IDG, and provide compassionate, patient-centered hospice care. Responsibilities include assessing needs, establishing goals and medical plans with physicians, and maintaining strong communication with patients, caregivers, and healthcare professionals.
