Home Health Speech Language Pathologist - per visit

BAYADA Home Health Care is seeking a Home Health Speech Language Pathologist for a job in Burlington, Vermont.

Office location:
Williston, Vermont. This office services adult and geriatric patients in homes throughout Chittenden County. The role involves home health visits as part of a multidisciplinary team to provide rehabilitative care and skilled nursing to clients who remain at home.

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations to assess, diagnose, and provide therapeutic interventions for clients experiencing speech, language, communication, or swallowing difficulties.
• Develop a treatment plan based on evaluations, physician orders, and recommended goals; continually assess and revise the speech therapy care plan.
• Participate with BAYADA clinicians in the multidisciplinary care plan as appropriate.
• Educate and instruct patients, family members, or other patient representatives in rehabilitative care and activities to promote health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided, utilizing provided electronic documentation tools.

• Current license as a Speech Language Pathologist in Vermont.
• Certificate of Clinical Competence in Speech from the American Speech-Language-Hearing Association (ASHA), or equivalent educational requirements and work experience toward certification, with ongoing certification process as applicable.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.

• Full-time, part-time, or per diem (staff).
• Ongoing position with BAYADA Home Health Care.
