Certified Diabetes Educator Registered Nurse

The University of Vermont Health Network seeks a Certified Diabetes Educator Registered Nurse to provide evidence-based diabetes self management education to diverse patients across Vermont and northern New York. In this collaborative, academic health system, you will assess learning needs, develop individualized education and care plans, support insulin and medication management, and coach patients on nutrition, lifestyle, and technology such as glucose monitors.

You will partner with endocrinology, primary care, and allied health teams, document in the EHR, and contribute to quality improvement, research, and community health initiatives in a culturally inclusive, patient centered environment.
