Admissions Nurse (RN)

We are currently seeking a Registered Nurse (RN) to join our Admissions team. If you are organized, confident, and passionate about communication and coordination, this could be a perfect fit.

Key Responsibilities
  • Conduct clinical screening of prospective residents.
  • Communicate with referral sources, families, and facility teams.
  • Collaborate with interdisciplinary teams to coordinate admissions.
  • Maintain accurate and timely documentation.
  • Represent Elderwood’s values and services in a professional, compassionate manner.
  • Assume responsibility for admission screenings and document compilation at local hospitals, ACFs, and resident homes.
  • Submit a statistical monthly report to the Director of Admissions.
  • Use voicemail and two‑way pagers for communication.
  • Visit designated hospital assignments daily.
  • Maximize productivity through communication with the Director of Admissions for re‑assignment or assignment of secondary support.
  • Participate in targeted marketing projects.
  • Initiate physician, hospital and referral source marketing efforts.
  • Coordinate pre‑admission planning programs.
  • Coordinate timely transfer of residents to Elderwood sites.
Qualifications
  • Active RN license in Vermont (or eligible for reciprocity).
  • Minimum 18 years of age required.
  • High school diploma or equivalent. Baccalaureate degree in Social Work, RN, or health‑related field required.
  • 2–3 years of clinical experience required.
  • Long‑term care, hospital discharge planning experience required.
  • Strong clinical assessment skills and working knowledge of medical charts and terminology.
  • Strong communication and organizational skills.
  • Comfortable working independently and collaboratively.
  • Compassionate, resident‑first mindset.
  • Valid state driver’s license and clean driving record; personal transportation.
  • Ability to read, write, speak, and understand English at an intermediate or more advanced level.
