Responsibilities
  • Deliver comprehensive dialysis care for ESRD patients in the inpatient/outpatient dialysis setting, including monitoring, assessments, and treatment administration per nephrology orders.
  • Train and maintain patients on home dialysis therapies, including education and ongoing support; operate Baxter PD cycles and NxStage home hemodialysis equipment as part of patient care.
  • Administer prescribed medications, manage IV access, monitor fluid status and laboratory parameters; uphold sterile technique and infection control standards.
  • Collaborate with the nephrology team to develop and adjust patient care plans; document care and outcomes in the EMR (Epic) per policy.
  • Maintain patient safety, respond to dialysis‑related emergencies, and ensure continuity of care across shifts.
Requirements
  • Active RN license to practice in Vermont
  • BLS certification
  • Home dialysis experience in an outpatient setting, including training patients on home dialysis and hands‑on experience with Baxter PD cycles and NxStage systems
  • Minimum 1 year outpatient/ESRD experience with chronic renal patients
  • Epic EMR experience within the past 2 years
  • Ability to work 8‑hour day shifts; no weekend work; flexible scheduling and reliable transportation required
