  • Clinical Oversight & Medical Care
    • Provide ongoing nursing assessment, monitoring, and care for program participants
    • Administer medications and maintain accurate, timely, and compliant documentation
    • Perform treatments including wound care and other clinical interventions as needed
    • Monitor and implement prescribed diets, medical protocols, and health-related supports
    • Identify changes in participant health status and communicate concerns promptly to supervisors and Interdisciplinary Team (IDT) members
    • Facilitate and oversee all medical procedures in accordance with physician orders and agency policies
  • Individualized Service Planning (ISP)
    • Support the development, implementation, and monitoring of ISP goals and health-related outcomes
    • Coordinate and align daily program activities with participant medical and behavioral needs
    • Document participant progress and health-related information in the electronic health record (e.g., Therap) including the completion of the Special Needs Checklist for all participants with a medical acuity.
  • Training & Staff Development
    • Provide medical and health-related training to direct support staff, including medication administration and recertification
    • Educate staff on participant-specific medical needs, dietary requirements, and care protocols
    • Participate in orientation and onboarding of new staff
  • Participant Support & Engagement
    • Promote a respectful, person-centered environment that upholds participant dignity, choice, and independence
    • Assist participants with activities of daily living (ADLs), including toileting, feeding, and personal care as needed
    • Support participant involvement in recreational, social, vocational, and community-based activities
  • Safety & Emergency Response
    • Respond to medical emergencies in accordance with Danielle’s Law and agency protocols, including activation of emergency medical services (911)
    • Ensure compliance with reporting requirements under Komninos’ Law for incidents involving abuse, neglect, or exploitation
  • Collaborate with Day Program Supervisors, residential nursing staff, and IDT members to ensure continuity and quality of care
  • Participate in interdisciplinary meetings and case reviews
  • Provide clinical oversight and consultation across programs when needed, including those staffed by LPNs
  • Quality Assurance & Oversight
    • Perform monthly record reviews for all participants with and without medical acuities
    • Ensure all medical documentation and practices meet regulatory and agency standards
  • Additional Duties
    • Perform other duties as assigned to support program operations and participant care
  • Schedule is M-F 8:00am-4:00pm

Requirements
  • Bachelor’s Degree in Nursing (BSN) or equivalent required
  • Current, active Registered Nurse licensure in the State of New Jersey
  • Minimum of three (3) years of nursing experience required
  • Experience supporting individuals with intellectual and developmental disabilities preferred
  • Valid driver’s license with an acceptable driving record in accordance with agency insurance requirements
  • Ability to meet physical demands of the position, including lifting, bending, and assisting individuals with mobility and activities of daily living
  • Current certification in CPR and First Aid, including ability to perform interventions at ground level

Summary

The Registered Nurse (RN) is responsible for providing comprehensive nursing services, medical oversight, and health-related supports to adults with intellectual and developmental disabilities participating in Community Access Center (CAC) Day Programs. The RN ensures the delivery of care in accordance with Individualized Service Plans (ISPs), Medicaid waiver requirements, and all applicable regulations set forth by the New Jersey Division of Developmental Disabilities (DDD) and the Department of Human Services (DHS).

The RN promotes the health, safety, and well-being of all participants while ensuring compliance with regulatory standards, agency policies, and best practices in developmental disabilities services.
