RN MDS Coordinator

Stellar Health Group is seeking a highly skilled RN MDS Coordinator to join our team.

About the Role:

As a RN MDS Coordinator with Stellar Health Group, you will play a critical role in ensuring the highest quality care for our patients. You will be responsible for coordinating the development and implementation of the Minimum Data Set (MDS) assessments, as well as conducting skilled care assessments and developing care plans. You will work closely with our interdisciplinary team to provide comprehensive care to our patients, and will be responsible for maintaining accurate and timely documentation of patient information.

Responsibilities:
  • Coordinate the development and implementation of MDS assessments, ensuring compliance with Medicare and Medicaid regulations
  • Conduct skilled care assessments and develop care plans in collaboration with the interdisciplinary team
  • Maintain accurate and timely documentation of patient information, including MDS assessments and care plans
  • Collaborate with healthcare providers to ensure continuity of care and communication
  • Identify and address any potential issues or concerns related to patient care or MDS assessments
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements:
  • Current RN license in the state of Vermont
  • Experience in long-term care or post-acute care settings
  • Knowledge of MDS assessments and regulations
  • Excellent communication and interpersonal skills
  • Strong organizational and time management skills
