Premier Rehab & Healthcare at Burlington is seeking a dedicated and compassionate Registered Nurse (RN) to join our team of healthcare professionals. As an RN, you will provide high-quality nursing care, lead nursing personnel, collaborate with an interdisciplinary team, and help ensure the health, safety, and well-being of our residents.
Responsibilities- Direct the daily activities of Certified Nursing Assistants in accordance with current federal, state, and local regulations, guidelines, and facility policies and procedures.
- Ensure nursing personnel comply with established policies and procedures.
- Participate in resident admissions, discharges, and transfers as required.
- Monitor residents for changes in condition and notify the physician and resident's family or representative as appropriate.
- Accurately document resident status and changes in condition.
- Transcribe physician orders to medical records and carry out orders as written.
- Prepare and administer medications according to physician orders and monitor residents for adverse effects.
- Perform wound treatments as ordered, monitor for changes, and document accordingly.
- Schedule resident follow-up appointments and transportation needs as indicated.
- Provide nursing leadership and guidance to personnel assigned to the unit.
- Provide direct nursing care, including colostomy changes, tube feedings, wound care, suctioning, IV administration, and other clinical services in accordance with facility policies and procedures.
- Perform regular rounds to ensure resident needs are being met and assigned duties are completed.
- Collaborate with members of the interdisciplinary team to ensure residents' physical, emotional, and clinical needs are addressed.
- Communicate concerns or complaints to the appropriate departments and file grievances as necessary.
- Report incidents and unusual occurrences to the appropriate supervisor or nursing leadership and participate in investigations as needed.
- Participate in end-of-life care as required.
- Monitor medication, supply, and equipment inventory and notify appropriate personnel when additional resources are needed.
- Initiate, review, and update resident care plans as required.
- Promote teamwork, mutual respect, and effective communication among staff.
- Participate in the survey process when regulatory inspection surveys occur.
- Current, unencumbered Vermont RN license.
- Strong communication and interpersonal skills.
- Ability to communicate effectively with residents, families, and staff.
- Strong critical-thinking and clinical decision-making skills.
- Ability to handle challenging situations with professionalism, patience, and tact.

- Ability to work collaboratively as part of an interdisciplinary team.
